What This Procedure Involves
In the emergency department, the wound is photographed, covered with a saline-soaked dressing, and antibiotics are given immediately. In theatre, the wound is extended and thoroughly debrided — all contaminated and non-viable tissue, including dead muscle and bone fragments, is removed. The wound is washed with litres of saline (irrigation). The fracture is stabilised — typically with external fixation initially, with conversion to internal fixation (nail or plate) once the soft tissue is clean. Wound management depends on the injury: primary closure, delayed closure, negative pressure wound therapy (VAC), or flap coverage for large soft tissue defects. Multiple operations may be needed.
