Knee arthroscopic debridement for arthritis involves washing out the knee and tidying up loose flaps of cartilage, removing loose bodies, and trimming meniscal tears in a knee with underlying osteoarthritis. Its role is limited and carefully selected.
Performed arthroscopically through two to three small incisions. The joint is irrigated (washed out). Loose bodies are removed. Unstable cartilage flaps are trimmed. Degenerate meniscal tears are debrided. Impinging synovium or plica are excised. The operation takes 20–40 minutes.
Arthroscopic debridement for arthritis has a very limited role. It may be considered for specific mechanical symptoms (true locking from a loose body or an unstable meniscal flap) in a knee with arthritis, when the mechanical symptom is clearly the predominant problem. It is NOT recommended as a general treatment for osteoarthritis.
Physiotherapy, weight management, analgesics, corticosteroid or hyaluronic acid injection, unloader bracing, and activity modification. These are the primary treatments for knee osteoarthritis and are more effective than arthroscopic debridement for most patients.
X-rays confirming arthritis. MRI showing a specific treatable mechanical lesion (loose body, unstable meniscal flap). Clinical correlation — the mechanical symptom must be the dominant complaint.
Relief of specific mechanical symptoms (catching, locking from loose bodies). Does NOT treat the underlying arthritis. Patients must understand that arthroscopic debridement does not slow arthritis progression, restore cartilage, or provide long-term pain relief from arthritis itself.
If the pain is primarily from arthritis rather than a mechanical lesion, debridement will not help.
The knee may be more painful and swollen for one to two weeks after arthroscopy.
Any benefit may be temporary.
Some patients feel worse after arthroscopy on an arthritic knee.

No. Arthroscopic debridement does not treat osteoarthritis. High-quality studies (the MOSAIC and Kirkley trials) showed no benefit of arthroscopic lavage/debridement over sham surgery or physiotherapy for osteoarthritis. It is only appropriate for specific mechanical symptoms in a knee that happens to also have arthritis.
It is only appropriate in carefully selected patients with a clear mechanical problem (a loose body causing true locking, or a flap tear causing catching) where the mechanical symptom is the dominant complaint. It is not appropriate as a general treatment for knee arthritis.
