What This Procedure Involves
A lateral (column procedure) or combined medial-lateral approach is used. The contracted anterior capsule is released. Coronoid and olecranon osteophytes are removed. Heterotopic ossification is excised. The posterior capsule may be released. The ulnar nerve is identified and transposed anteriorly (moved to the front of the elbow) to protect it as the elbow is mobilised. The elbow is taken through a full range of motion intra-operatively. Prophylaxis against recurrent HO (NSAIDs and/or single-dose radiation) is given. The operation takes one and a half to two and a half hours.
