Elbow Contracture Release (Open / Column Procedure)

⚙️What This Procedure Involves

A lateral (column procedure) or combined medial-lateral approach is used. The contracted anterior capsule is released. Coronoid and olecranon osteophytes are removed. Heterotopic ossification is excised. The posterior capsule may be released. The ulnar nerve is identified and transposed anteriorly (moved to the front of the elbow) to protect it as the elbow is mobilised. The elbow is taken through a full range of motion intra-operatively. Prophylaxis against recurrent HO (NSAIDs and/or single-dose radiation) is given. The operation takes one and a half to two and a half hours.

💡Why It May Be Recommended

Open release is recommended for severe elbow contracture (functional arc less than 100 degrees — less than 30-130 degrees) that has failed physiotherapy and serial splinting, particularly when there is significant heterotopic ossification, ulnar nerve symptoms, or when the anatomy makes arthroscopic release unsafe.

⚖️Non-Operative Alternatives

Intensive physiotherapy, static progressive splinting (JAS or Dynasplint), serial casting, and arthroscopic capsular release for milder contractures without HO.

📋Preparing for Surgery

CT scan to map heterotopic bone and plan the approach. Nerve conduction studies if ulnar nerve symptoms. Ensure HO is mature (at least six to twelve months from injury) — premature excision increases recurrence risk.

Improvement of approximately 40–60 degrees of total arc. Most patients achieve a functional arc (30–130 degrees). Maintaining gains requires immediate, aggressive post-operative physiotherapy.

💉Anaesthetic Considerations

General anaesthesia with continuous regional nerve block (catheter) for 48–72 hours of pain-free intensive exercises.

🏃Recovery and Rehabilitation

Continuous passive motion (CPM) machine begins immediately in recovery. Regional block catheter provides pain relief for intensive exercises. Physiotherapy three times daily for the first week. Splinting between exercise sessions. NSAIDs or single-dose radiation for HO prophylaxis. The first two weeks are critical — gains made in this period determine the long-term outcome. Full recovery three to six months.

Frequent early reviews — weekly for the first month. X-ray at six weeks and three months for HO surveillance.
