Elbow aspiration is a procedure to remove fluid from the elbow joint using a needle and syringe. It is performed diagnostically (to analyse the fluid) or therapeutically (to relieve a tense effusion and inject corticosteroid).
The skin is cleaned with antiseptic. The elbow is positioned at 90 degrees of flexion. A needle is inserted into the joint through the 'soft spot' — the triangle between the olecranon, lateral epicondyle, and radial head. Fluid is aspirated. If therapeutic, corticosteroid and local anaesthetic are injected. The procedure takes 5–10 minutes.
Aspiration is performed for diagnosis of septic arthritis (emergency), crystal arthropathy (gout or pseudogout), inflammatory arthritis, and to relieve a painful tense effusion after injury or surgery.
Blood tests and imaging can support diagnosis but cannot definitively diagnose joint infection or crystals. When infection is suspected, aspiration is urgent and essential.
No fasting required. Can be performed at the bedside or in clinic.
Definitive fluid analysis for diagnosis. Therapeutic relief of a tense, painful effusion. If corticosteroid is injected, additional pain relief.
Brief pain during needle insertion.
Minor bruising.
Fluid may reaccumulate if the underlying cause is not treated.
Introduction of infection. Very rare with sterile technique.
Posterior interosseous nerve is nearby. Very rare.
Temporary pain increase after injection.
No anaesthesia needed beyond local skin anaesthesia.
No recovery needed. Rest the elbow for 24 hours after injection.
Fluid results within 24–48 hours. Follow-up based on results.
The soft spot is the triangle of soft tissue between the olecranon (elbow tip), lateral epicondyle (outer bony bump), and radial head. It provides safe, reliable access to the elbow joint.
